DANCE FX

RULES AND REGULATIONS: Please read and sign one copy and retain the other for your future reference

I UNDERSTAND:
1. that tuition for all classes is due by the 1st of each month. If payment is made after the 7th a late fee of $15.00 will automatically be charged to my account (unless other arrangements are made in advance).  If check is returned, a returned check fee of $30.00 will be added to my account.

2. that I will be responsible for the full monthly tuition of my child and that no refunds or credits will be given   
for missed classes. If and illness requires absence, a make-up class will be available until February. After February, make ups are only allowed it the first portion of class.

3. that an interest rate of 1.5% per month will be charged on all accounts past due for 30 days or more. I will be responsible for all legal and collection fees incurred by Dance FX should my account become delinquent.
 
4. that the school reserves the right to drop registrants from their class for:
a) excessive lateness and/or absence;
b) disruptive behavior;
c) non-payment of tuition or other charges;

5. that in the event of an accident or illness involving my child, I will be notified immediately. In the event Icannot be contacted, I hereby authorize Dance FX to take any steps it deems necessary to make medical attention available. Medical attention includes physician, hospitals, or any other medical services that Dance FX considers immediate.
      
6. that any limitations in ability to participate due to a medical condition must be noted on the registration form as “Health Information” and “Physical Limitations. Failure to note such conditions is a warranty by the parent that such conditions do not exist.

7. I hereby acknowledge that I have been informed by Dance FX of the nature of the instruction that my child will Receive and that such instruction involves physical exercise and stress which could result in injury.  It is further Agreed that unless caused by the negligence of the studio, I hereby waive all claims and do not hold Dance FX and/or it’s instructors responsible for any such injury incurred during the course of instruction or while waiting to be picked up.

8. that I will drop off and pick-up my child on time and I understand that Dance FX, and it’s employees are not responsible for children left before or after their class.

9. that the school reserves the right to alter class schedules, for any reason (either temporary or permanently), without fault.

10. that my child will be expected to participate in the year end concert production, unless I notify Dance FX by Nov. 1, 2005.

11. that costumes will be ordered and must be paid in full by November 1, 2005. Exchanges must be paid for by parents (NO REFUNDS). Tentative concert date 1st weekend in June. Confirmation to follow.

12. that Dance FX does not have a strict dress code for the student’s jazz or tap classes (no baggy shirts or pants). However, any student enrolled in a classical ballet class must wear the required ballet outfit: leotard made for Dance FX, pink tights, and pink split-sole ballet shoes. In addition, their hair must be worn in a bun and secured with a hair net. Necklaces, bracelets or long earrings are not allowed.

13. that Dance FX does not allow any student of any age to leave the studio without any adult. A designated adult will come to pick up my child. Parents are expected to pick-up their children immediately after class. If my child remains at the studio for more than 15 minutes without prior arrangements. I understand my account will be charged $10.00 fee per half hour. If the studio is closing, and arrangements have not been made, I understand that my child will be taken to the instructor’s home and a note will be left on the door.

14. that all recordings including, video, audio, still pictures, motion pictures of any event (including but not limited to concert, rehearsals, dance acts, classes and demonstration), remain the property of the studio with all rights of ownership retained by the studio.

     
15. that any questions will be handled by the office and not during the instructor’s class time.

16. that in order to purchase end of the year concert tickets, all accounts must be at zero balance.
